Open enrollment Form Submissions will be accepted 1/1/25 till 1/31/25. Submissions received after 1/31/25 will be declined. Submissions received before 1/1/25 will be declined. Your 12 week period will start the day you submit the form.

12 Weeks - Your 12 week period starts when you submit the form.

First - Place your order for the MindBody GLP-1 System. You may go on a subscription (best price) for 3 months. Proof of purchase may not be before 12/24/24 (challenge announced 12/23/24 to consultants). If you wish to purchase all 3 months at once you may do so however, the proof of purchase date must be on or after 12/24/24.

Second - Complete the “Activate and Thrive in ‘25: 12 Week MindBody Challenge” enrollment form between 1/1/25 - 1/31/25. Submitting the form means you agreed to the terms of the challenge and are ready to transform your health in 2025. IMPORTANT NOTE - Before photos and progress photos will be required. If you don’t wish to share photos, you will not be eligible for the cash prizes.

Third - Join your designated Facebook Group for the chance at weekly prizes during the challenge. Ask the person who shared this Challenge with you for “your” Facebook Group.

There will be 15 total Cash Prize winners. Five winners for each category. If you choose to compete in the couples category you can not compete in the individual categories;

Participating in the challenge means you agree to purchase 3 “MindBody GLP-1 Systems”. This gives you the product you’ll need for the 12 weeks. The product can be purchased all at once or for 3 months (on subscription). The first purchase date can not be before 12/24/24.

You’ll join “your Teams” designated Facebook Group. Refer to the person who shared this challenge with you for your “Designated” Facebook Group. If you want in on the weekly prize giveaways you must be in your designated FB group. If you don’t care to be in the weekly giveaways, you are not “required” to join a FB group. You’re still required to submit your progress results on weeks 4, 8 and 12 if you want to be eligible for the cash prizes.

You’ll submit a progress result Form three times over the course of 12 weeks; One at 4 weeks 8 weeks 12 weeks

All results/ transformation journey images and testimonies will become intellectual property of the Challenges administrators. Full disclosure can be found on the “Challenge Enrollment Form”.

What if more than one participant/ Lifevantage customer/ consultant account is in a household? For proof of purchase, the systems can be purchased on one account. You do not have to split the orders up for each participant within the same household. If you order separately, that is okay too.
